I was advised I had court orders for a reason so i could always pick them up from school. But my oldest is on medicine he can't be without. Plus I don't want them to be scared or worried or feel like they did something wrong

Sasha, what state are you in? Have you contacted your District Attorney's Child Abduction Unit? You may have the option to file a motion asking the Court to issue a protective custody warrant which the CAU can then execute, take the children into PC and give them to you. You may also concurrently request to modify the orders to sole legal and physical custody to you with father to have professionally supervised visitation at his expense. You have the option to file on an Ex Parte basis if your child is in danger. If you can't afford the court filing fee, file a Fee Waiver.
